| If you have been involved in a motor vehicle accident, please bring a
copy of the police report, all insurance information, and any x-rays or
doctor's reports. |
| If you have workman's compensation please bring carrier
name, address, phone and your claim number |
| Payment is due at the time of service. |
| Delays- On the day of your appointment there may be delays on patient
scheduling. Sometimes the delays are necessary so that we can spend efficient
time with each patient to make sure we take care of their plan of treatment. We
