INTRODUCTION

The Spine and Brain Neurosurgical Center was founded in May 1995 by John W. Gilbert, M.D., a fourth generation Kentucky private practice physician with board certification status by the American Board of Neurological Surgeons, American Academy of Pain Management, American Board of Pain Medicine, American Society of Neuroimaging with an emphasis on MRI and CT of the nervous system, and the American Society of Addiction Medicine, and the Medical Review Officer Certification Council (certified medical review officer MRO).  The practice has grown and now has just hired the 10th doctor (7 are M.D.’s, 2 are D.O's., and another a Ph.D.), and these physicians have various specialties including neurosurgery, pain management, pain medicine, neuroimaging, addiction medicine, neurology, psychiatry, neuroradiology, radiology, musculoskeletal medicine, quality assurance, and a Ph.D. in physical therapy.  The practice has 5 locations including Lexington, Florence, Pikeville, Hazard, and London, Kentucky which serves essentially central and eastern Kentucky as well as many patients from northern Tennessee, western portion of West Virginia, and southern portion of southern Ohio.  Patients are only seen at the request of their personal physician and require a referral for evaluation and treatment and consultation, and currently the practice has over 2000 referring physicians that request services.  The physicians are supported by over 60 employees including in-house counsel, CEO, administrative staff, on-site managers, nursing staff, medical assistances, technologist, marketing personnel, pre-certification staff, billing and collection, coding staff, telephone triage and call center, and a myriad of other vendors and consultants.  The integration of practice management technology, office management, finance, personnel, and patient and referring physician satisfaction has allowed the practice to succeed in its mission to provide state of the art of multiple specialty neuroscience care close to the home of many patients and referring physicians in rural Appalachia and other underserved areas.  The founder of the practice is able to practice neuroimaging every day in each office by reading MRI exams from each location of the nervous system utilizing dedicated T-lines and Radworks™ technology.  In addition, the practice has received approval to expand telemedicine into psychiatry, addiction medicine, and other areas of the practice as indicated.  With the help of our referring physicians, our neuroscience physicians and staff the practice has achieved numerous goals including being the first practice in the world to receive five separate pain program accreditation awards from the American Academy of Pain Management, the first neurosurgery office in the world to offer positional MRI imaging, and the practice also has been able to achieve the American College of Radiology (ACR) accreditation for each location for MRI imaging and in fact is the first and only practice in Kentucky to offer ACR accredited MRI imaging in Florence, London, Pikeville, and Hazard, Kentucky.  The resulting improved patient care, access to care, improved referring physician satisfaction, and other factors have resulted in the founder of the practice being selected for inclusion the past four years in Guide to America’s Top Surgeons and Guide to America’s Top Physicians by the Consumers’ Research Council of America, as well as International Health Professional of the Year Award by the International Biographical Center, Cambridge, England 2006.
