John W. Gilbert, M.D.John W. Gilbert, M.D. is a 4th generation Kentucky private practice physician who completed his training in neurosurgery at the University of Kentucky in 1990. He has been in private medical practice since 1990 and founded the Spine and Brain Neurosurgical Center in 1995. He has been chief of the neurosurgery division at Samaritan Hospital since 1997 and St. Joseph East Hospital since 1999, and is presently co-chief of neurosurgery at Select Specialty Hospital. He also has attending privileges at Central Baptist Hospital and St. Joseph Hospital. He is an assistant professor at the University of Kentucky College of Medicine and professor of Neurosurgery and professor of Pain Management at the Pikeville Medical School. He has achieved board certification status from the American Board of Neurological Surgery, American Academy of Pain Management, American Board of Pain Medicine, and is the only neurosurgeon in Kentucky to have board certification status from the American Society of Neuroimaging with a special emphasis on MRI and CT of the nervous system. He is one of 11 Kentucky physicians to achieve board certification status in addiction medicine. He implemented the first and only American College of Radiology (ACR) accredited MRI neuroimaging in Hazard, London, Pikeville and Florence, Kentucky, and received congratulations from the inventor of the MRI for being the first neurosurgery office in the world to offer upright positional imaging. He practices neuroimaging in all offices on a daily basis via telemedicine/teleradiology. He and the other multispecialty neuroscience physicians have published in national peer reviewed journals, and this has helped lead the practice to becoming the first in the world to receive five separate pain program accreditation awards. He was selected to “Guide to America’s Top Surgeons” and “Guide to America’s Top Physicians” in 2006 and 2007 as well as previous years by the Consumers’ Research Council of America. He was recently awarded International Health Professional of the Year by the International Biographical Center of Cambridge England. He is a Certified Medical Review Officer (MRO) through the Medical Review Officer Certification Council (MROCC), based upon Federal requirements developed by the Dept of health and Human Services for Federal Drug Testing Programs.
Greg R. Wheeler, M.D.Greg R. Wheeler, M.D. completed his neurosurgery residency and training at the University of Louisville and has been in private practice with Dr. Gilbert since 1997. He and Dr. Gilbert initially covered over 13 clinics a month at separate locations until being able to consolidate to the 5 present clinics in Florence, Hazard, London, Pikeville, and Lexington. He is an attending neurosurgeon at St. Joseph Hospital, Central Baptist Hospital, St. Joseph East Hospital, and is former chairman of the entire department of Surgery at Samaritan Hospital. He is co-chief of Neurosurgery at Select Specialty Hospital. He has obtained board certification status and is a diplomat of the American Board of Neurological Surgery. He is an assistant professor at the University of Kentucky College of Medicine. He has an interest in complex disorders of the spine and brain, and performed the first lumbar I/F cage fusion in Lexington, Kentucky. In addition he was the first neurosurgeon at the Spine and Brain Neurosurgical Center to utilize intraoperative Gliadel wafers (chemotherapy wafers) for the treatment of malignant glioblastoma of the brain. In addition, he has become the first neurosurgeon here to utilize the X stop which is a new FDA approved minimally invasive procedure for spinal stenosis. He was the first neurosurgeon here to implement the use of state of the art programmable implant systems for the treatment of complex normal pressure hydrocephalus and other disorders. He was the first neurosurgeon here to implement and utilize the NIM-Spine System to analyze neurointegrity of the spinal cord, brain, cranial nerves, and peripheral nerves. He has developed special expertise in a wide spectrum of complicated neurological disorders requiring different types of implanted devices to treat specific underlying multi-specialty neuroscience pathologies. His technical skills as an operative neurosurgeon and patient outcomes have resulted in not only physicians from Kentucky requesting consultations but those from the surrounding states and further requesting his neurosurgical expertise. He is certified with the first FDA approved artificial disc (CHARITE’) from DePuy and is the first Surgeon in Lexington, Kentucky to place the first approved Artificial Disc for the Cervical Spine.
Shailesh P. Upadhyay, M.D. graduated from medical school in 1975 from the University of Bombay in Bombay, India, and completed a rotating internship at Kem Hospital in India. He completed an anesthesiology residency at the Medical College of Virginia in Richmond, Virginia July 1977 through June 1980. He has obtained board certification status in pain management through the American Academy of Pain Management and is pursuing board certification status in addiction medicine, as well as has become a member of numerous medical societies. He has served as chief of anesthesia at North River Surgical Center in Tuscaloosa, Alabama as well as president of the Coosa Valley Anesthesia Associates in Sylacauga, Alabama, and has been in private practice since residency. He has over 30 years of practical experience in anesthesia and pain management as well as over 30 years experience in injective therapies such as epidural blocks to treat acute and chronic pain. He has co-authored protocols on universal precautions for treating chronic non cancer pain patients as well as developed expertise in individualized diagnosis and treatment recommendations within standard guidelines, he has provided valuable patient care for patients referred to the Spine and Brain Neurosurgical Center since August 2005. He resides in Pikeville and covers other clinics as needed. He has been able to help many neurosurgical patients improve with conservative treatment recommendations and injective therapy close to home and avoid surgery. He has recently completed extensive CME in addiction medicine and new procedures in pain management.
Benjamin B. Storey, M.D. attended the University of Kentucky and graduated with distinction as well as becoming a Rhodes Scholar candidate. In addition, he obtained a medical degree from Cornell University Medical College. He performed medical internship at Bellevue Hospital Cornell Division of Slone Kettering Memorial Hospital and became a fellow in internal medicine at the Mayo Clinic. He also completed a residency in psychiatry at the University of Cincinnati. He was a captain in the US Army Medical Corp and worked at the Cincinnati VA Hospital Department of Psychiatry and started a complete 32 bed in-patient and large out-patient drug treatment center. He has also been on faculty at the Department of Psychiatry at the University of Cincinnati as well as the faculty of the Department of Psychiatry at the University of Kentucky, and has been in private practice in psychiatry in Cincinnati, Ohio until 1972. He has been chief of the Department of Psychiatry at Good Samaritan Hospital in Lexington, Kentucky for most of his 30 years of practice and has been on numerous hospital staffs in Lexington, Kentucky throughout that time. In addition, he has worked at Eating Disorders and Substance Abuse Hospital in 2002 as well as Eastern State Hospital in Lexington, Kentucky. He has worked full time since January 2006 for the Spine and Brain Neurosurgical Center providing a full range of out-patient psychiatric services, evaluations and consultations including evaluation and treatment recommendation for potential substance use disorders including evaluation of patients with dual diagnoses and other complex behavioral health patients. He also provides evaluations of patients considering intrathecal pump or epidural stimulator implants as well as other procedures. He has been a key participant in the startup of the first private practice in Kentucky to offer telepsychiatry and teleaddiction medicine services with Dr. Gilbert. This allows Dr. Storey and Dr. Gilbert to practice psychiatry and addiction medicine respectively throughout all clinic offices via the Telehealth Network. He has been a welcome addition to our practice and wonderful resource for our physicians, patients, and referring doctors.
Dr. Martin Paul Kreft, DPT has achieved a Doctorate of physical therapy from Boston University, Boston, Massachusetts with honors in January 2005. He has also achieved Master of Science and Bachelor of Science at the University of Kentucky, and in addition has become certified as a physical therapist by the National Board for Certification in Physical Therapy in 1997. He was award the AEGIS Therapies Employee/Educator of the Quarter in 2003 for efforts in promoting the need for strength training and fall prevention in the geriatric population. He has served as director of physical therapy services and staff physical therapist at Memorial Hospital in Manchester, Kentucky as well as Jenkins Community Hospital in Jenkins, Kentucky. He has also served as master clinician for AEGIS Therapies at The Heritage as well as served as therapist at Sundance Rehabilitation. He has also served as the lead physical therapist for a neurosurgical clinic of Gateway Rehabilitation Services. He has provided physical therapy evaluation and recommendation services at the Spine and Brain Neurosurgical Center clinic since 2004. He sees patients only at the request of Spine and Brain Neurosurgical Center physicians, and provides myofascial and physical therapy diagnosis and treatment plan recommendations which the patient can then perform locally with their local chiropractor or physical therapist as well as extensive home exercise programs for the maintenance and improvement of a variety of patient neurological functions.
Richard D. Gibbs, M.D. obtained his medical degree from the University of Cincinnati College of Medicine in 1988, and performed and internship in internal medicine at Wilford Hall Medical Center in Texas through 1989, and a residency in Diagnostic Radiology at Wilford Hall Medical Center in Texas through 1995, and completed a fellowship in vascular and interventional radiology at Wilford Hall Medical Center in 1996. He has obtained board certification status by the American Board of Radiology Diagnostic Radiology in June 1995, and also obtained additional certificate of added qualification in vascular as well as interventional radiology in November 1998. He has served as a chief of Diagnostic Radiology at Wright Patterson Medical Center, Wright Patterson Air Force Base in Ohio as well as function as a flight surgeon. He has served as medical director at Mercy Medical Center in Springfield, Ohio as well as Mercy Memorial Hospital in Urbana, Ohio. He has been awarded the Alpha Omega Alpha accreditation by the University of Cincinnati College of Medicine. He is in numerous medical society memberships and has received awards, and provided numerous medical presentations and programs. In addition, has recently co-authored several neuroimaging based case reports with other physicians which have been submitted to national peer reviewed publications. He has provided part time back up p.r.n. neuroradiology and radiology consultation evaluation and recommendations at the Spine and Brain Neurosurgical Center, dba Open Stand Up MRI.com, since 2005.
Kwadwo Gyarteng-Dakwa, M.D. completed medical school at Bolgograd State Medical Institute in Russia June 1990, and then an internship at Pediatrics Seton Hall University Medical Center Patterson, New Jersey July 1995 through June 1997. He completed a fellowship and became a post doctoral fellow of the Department of Anesthesiology Yale University School of Medicine New Haven, Connecticut and served as an assistant professor at the Department of Anesthesiology Yale University July 2002 through June 2005. He completed his residency at Howard University Washington DC July 1997 through June 2001. He has extensive experience in anesthesiology, cardiology, pediatric, pain management, pain medicine, and has numerous honors and medical recognition certificates, and is a member of the American Board of Anesthesiologist as well as other medical organizations. He has extensive experience with spinal epidurals as well as other aspects of pain management while an assistant professor at Yale University Medical School in Department of Anesthesiology at Yale University Hospital from July 2002 through June 2005. He has provided anesthesiology, pain management and pain medicine evaluation, and consultations at the Spine and Brain Neurosurgical Center at various outreach clinics as well as Lexington, Kentucky since October 2005 since leaving his attending position at Yale University Medical School. He has provided part time p.r.n. back up services.
Dr. Clifton D. Burt obtained his medical degree from East Tennessee State University College of Medicine in 1996 and completed his internship at the Department of Internal Medicine at Mercy Fitzgerald Hospital in Darby, Pennsylvania in July 1998. He performed and completed a residency at the Department of Physical Medicine and Rehabilitation at Medical College of Virginia in Richmond, Virginia in July 2001 and completed a fellowship as well in Physical Medicine and Rehabilitation and Pain Management at the Medical College of Virginia in Richmond, Virginia in July 2002. He served as a contributing editor to the Publication, Rehabilitation and Review in 1999 and served as an assistant sports medicine director at the Medical College of Virginia in 1998 and 1999 as well as a team physician for Virginia Commonwealth University Athletic Department and a clinical musculoskeletal exam course instructor. He has diverse research interests and scientific presentations/publications such as "Aquatic Therapy Rehabilitation" @ Medical College of Virginia - 2000, "Rehabilitation for Patients with Multiple Sclerosis" @ Medical College of Virginia - 1999, and collaborated with D. Burnett regarding "The Effect of Hospital Insurance Carrier on the Length of Inpatient Hospitalization." He belongs to the American Academy of Physical Rehabilitation Medicine, National Medical Association, National Multiple Sclerosis Foundation Society, Physiatric Association of Spine, Sports, and Occupational Rehabilitation, and the American Medical Society. He is a retired Captain of the United States Army Reserve (2000) and has taught chemistry and physics at Armstrong School, John Marshall School, and the Franklin Military Academy. His experience in Physical Medicine and Rehabilitation, Pain Management, Neurosciences, research, teaching, and military make him a welcome addition to our multispecialty neuroscience practice.
Gay Best Richardson, M.D. completed a BS Degree in Biology with a minor in Chemistry at Eastern Kentucky University in 1993 and completed her medical degree at the University Of Kentucky College Of Medicine in 1997. She is a member of the American Academy of Pain Medicine, American Academy of Physical Medicine and Rehabilitation, American Medical Association, Kentucky Medical Association, and other prominent organizations. She has served as Chief Resident as well as in the Neurology Residency Review Committee as well as Cardiology and Family Practice Residency Review Committee. She has supervised and taught medical students as well as served on the Medical Quality Improvement Committee at Cardinal Hill Rehabilitation Hospital. She has extensive research experience and extensive formal presentations and lectures in the Neurosciences. She has received numerous awards and honors including the David L. Cohen Award and graduated Magna Cum Laude as well as other commendations. She completed her Residency in Physical Medicine and Rehabilitation at the University of Kentucky, Chandler Medical Center, Cardinal Hill Rehabilitation Hospital, Veterans Affairs Medical Center, Lexington Center For Health and Rehabilitation and the Shiners Hospital for Children. She has achieved board certification status by the American Board of Physical Medicine and Rehabilitation in 2002. She has also achieved board certification status by the American Board of Pain Medicine in 2004, and the American Board of Electrodiagnostic Medicine in 2006. She has extensive experience as an attending physician in Neurosurgery practice and has special interest in neck and low back pain, myofascial pain syndrome, electrodiagnostics, injective therapy and other neuroscience diagnoses. Her multiple board certifications in Physical Medicine and Rehabilitation, Pain Medicine, Electroneurodiagnostic medicine and her extensive clinical and research experience make her a welcome addition to our multispecialty neuroscience practice both for our physicians, our referring physicians, and our mutual patients.
Stephanie Lynn Herder, MD Stephanie Lynn Herder, M.D. is a board certified neurosurgeon and diplomat in the American Board of Neurological Surgery who attended medical school at the Health Sciences Center at St. John's in Newfoundland. She completed a general surgery neurosurgery residency at the Memorial University of Newfoundland Teaching Hospitals and the University of Ottawa Teaching Hospital School of Medicine in 1985. She also completed a fellowship in Pediatric Neurosurgery in 1986 at the University of Ottawa. She has an extensive clinical and research experience in general and pediatric neurosurgery and in addition has published in the prestigious peer reviewed journal Child's Nervous System as well as the prestigious Journal of Neurosurgery. She has numerous degrees, diplomas and certificates including a Doctor of Medicine, a specialist certificate in Neurosurgery from the Royal College of Physicians and Surgeon of Canada in 1987. She is a fellow of the Royal College of Physicians and Surgeons of Canada in 1988 and received diplomat and board certification status by the American Board of Neurological Surgery in 1993. She has been a member in the past of the American Association of Neurological Surgeons, Kentucky Medical Association, American Medical Association, Montana Medical Association, and is presently a member of the Congress of Neurological Surgeons. She has had extensive experience at a level I trauma center with 80% cranial neurosurgery procedures. Dr. Herder's remarkable accomplishment of obtaining a board certification status both from the American Board of Neurological Surgery as well as the Royal College of Surgeons in Neurosurgery and her extensive experience in general pediatric and craniospinal neurosurgery make her a welcome addition to our multispecialty neuroscience practice both for our physicians, referring physicians and our mutual patients.
John E. Clontz received his Bachelor of Arts in economics from Centre College in Danville, Kentucky in 1979 and is jurist doctor from the University of Kentucky in 1982. He was a partner at Clontz & Cox Attorneys from 1982 through 2003. He is a member of the Kentucky Bar Association and the American Health Lawyers Association. He has served as chairman of the Rockcastle County Development Board from 1986 through 2002 and developed a 10 year county strategic plan and county leadership training program patterned after Leadership Kentucky now in its third year. He has been chairman of the Rockcastle County School District Strategic Planning Committee and developed a seven year strategic consolidation and building plan for school district. He has received the Everett C. Brown Award for community leadership. He has served as general counsel and corporate secretary for Health Essential Solutions Incorporated from March 2003 through November 2005. He was responsible for all legal and administrative matters arising from the company’s nurse practitioner, physician house calls, home health and Hospice businesses in 15 states. His responsibilities included organizing and qualifying operating units and obtaining and maintaining all federal, state, and local licensure requirements, negotiation of operation contracts, human relation labor matters, oversight of HR department, management of all legal proceedings, management of all Medicare and Medicaid matters, and served as the company compliance officer. John has worked full time for Physician Services, DBA Spine and Brain Neurosurgical Center, DBA Open/StandupMRI.com as full time in-house counsel and full time chief compliance officer since 2005. He has also functioned as an administrator for the practice since then as well, and is responsible for all legal, regulatory, compliance, contracts, as well as myriad of other administrative and legal functions for the Spine and Brain Neurosurgical Center. His responsibilities have also included management of all Medicare, Medicaid, Workman's Compensation, or any other commercial insurance matters and compliance. In addition, he oversees Spine and Brain Neurosurgical Center services and reviews and insures compliance with all applicable local, state, and federal statute. His extensive experience in previous health care company in 15 states has made him a welcome addition to the Spine and Brain Neurosurgical Center.
Dr. Mick graduated from the Kansas City College of Medicine in 1980 and completed internship at Normandy Hospital in 1981. Dr. Mick worked in Family Medicine in Hardin, Illinois as well as Jerseyville Primary Care Associates from 1981 through 1991. He completed Residency in Neurosurgery at Deaconess Medical Center in St. Louis, Missouri from 1991 through 1996. He also completed a Fellowship in Neurosurgical Trauma in 1995 at Allegheny General Hospital in Pittsburgh, Pennsylvania. He has extensive experience as an attending neurosurgeon at the well-known Neurosurgery and Spine Associates of Alabama, Central Washington Neurosciences of Washington, and the Trover Clinic of Kentucky. He is the founder and president of the Pacific Northwest University of Health Sciences in Washington. He is a member and affiliated with numerous organizations including the American Medical Association, the American College of Osteopathic Surgeons, and other medical and neurosurgical organizations. He has honors including performing as a medical examiner for the Federal Aviation Administration as well as a Neurosurgical Board Examiner for the American Board of Osteopathic Neurosurgery, and has been Chairman of the Board of Pacific Northwest University of Health Sciences as well as directorships, awards committees, and has been president and vice president of Medical Staff as well as Director of Medical Education at Yakima Regional Medical Center. He has published in peer reviewed journals as well as peer reviewed presentations on neurosurgical issues as diverse as spinal epidural hematomas and extended transsphenoidal approach for resection of parasellar craniopharyngiomas and skull base reconstruction. Our referring primary care doctors will appreciate Dr. Mick’s 10 years of experience in Family Practice and our neurosurgery and neuroscience physicians and patients appreciate his neurosurgical expertise and acumen that have qualified him to become a Neurosurgery Board Examiner. His experience with complex neurosurgical and neuroscience issues make him a welcome addition to our practice.
M. Steven Hanson is a native of Kentucky. He attended Centre College and received his civil engineering degree from the University of Kentucky. He continued his education through the Life Underwriters Training and Council, College for Financial Planning, and is a graduate of the Tax Institute on Estate Planning held at Colorado State University. He is president and founder of Pinnacle Planning and is involved in financial services industry since 1990. He is a member of the National Association of Insurance and Financial Advisers and is a qualifying member of the Million Dollar Round Table. He is a registered representative with W. R. P. Investments Incorporated, and holds a Series 6, 7, and 63 securities licenses. He has been a frequent speaker at both publicly and privately sponsored seminars on estate, investment, insurance, and business planning. He has worked with the Spine and Brain Neurosurgical Center since it was founded in 1995 by Dr. Gilbert as a practice consultant. He has assisted in human resource development, financial planning for the practice as well as strategic business planning for the practice, and has been instrumental in consulting with the administration of the practice. He has also assisted in public relations, and spearheaded and developed state of the art cost effective pension profit sharing retirement plans for the employees as well as health and dental benefits, vision benefits, short and long term disability, and life insurance benefits as the practice has grown from one employee John W. Gilbert, M.D., PSC in 1995 to the current multispecialty neuroscience practice with 15 doctors and a total of approximately 70 employees. He has helped implement state of the art 200 bit encrypted T-lines with the practices own intranet and electronic medical records system and voice over intranet telephony system to further leverage the skills of the physicians and staff of the Spine and Brain Neurosurgical Center. He has helped engineer continuous quality control processes thus allowed the practice to continue to grow and assisted the physicians in providing state of the art multispecialty cost effective neuroscience care.
